By downloading, installing, or using DoseForm, you agree to these Terms of Use. If you do not agree, do not use the app.

Description of Service

DoseForm is a personal health tracking tool for logging peptide protocols, doses, reconstitution calculations, and related health metrics. It is designed as a personal reference and logging tool only.

Medical Disclaimer

DoseForm is not a medical device and does not provide medical advice, diagnosis, or treatment. Estimated drug levels, reconstitution calculations, and all other data presented in the app are for informational and personal tracking purposes only. Always consult a qualified healthcare provider before starting, modifying, or stopping any medication or supplement protocol. Do not rely on DoseForm for clinical decision-making.

Subscriptions & Billing

DoseForm Premium is available as a monthly subscription, annual subscription, or a one-time lifetime purchase. Current pricing is displayed in the app. Prices may vary by region and are subject to change.

• Payment is charged to your Apple ID account at confirmation of purchase.

• Subscriptions (monthly and annual) automatically renew unless cancelled at least 24 hours before the end of the current period.

• Your account will be charged for renewal within 24 hours prior to the end of the current period at the same price.

• You can manage and cancel subscriptions in your device Settings > Apple ID > Subscriptions.

• Any unused portion of a free trial period is forfeited when you purchase a subscription.

• The Lifetime purchase is a one-time, non-recurring charge that grants permanent access to all premium features.

• Refunds for subscriptions and purchases are handled by Apple per their refund policy.

User Responsibilities

You are responsible for the accuracy of data you enter into DoseForm. You are responsible for maintaining the security of your device and any data stored on it.

Intellectual Property

DoseForm, its design, code, and content are the intellectual property of the developer. You are granted a limited, non-transferable license to use the app for personal, non-commercial purposes.

Limitation of Liability

To the maximum extent permitted by law, DoseForm and its developer shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of the app, including but not limited to health outcomes, data loss, or service interruptions.

Termination

We reserve the right to terminate or restrict access to DoseForm at any time, with or without notice, for conduct that we believe violates these Terms or is harmful to other users or the service.

Changes to Terms

We may update these Terms of Use from time to time. Continued use of DoseForm after changes constitutes acceptance of the revised terms.
